A comprehensive regular evaluation at a single multidisciplinary clinic supports a personalized treatment plan. Specialists include cardiologists, hepatologists, nephrologists, nutritionists and psychologists.
Who to Refer
Children and young adults up to age 22 who have completed Fontan palliation, require routine surveillance or have known or suspected Fontan-associated complications.
Collaborative Care
The program complements—not replaces—ongoing care provided by the patient's cardiologist. Recommendations follow each evaluation.
